Medical billing and coding is the process of computer aided centralization of a coding system, accurate electronic recording and data entry processing of a patient's symptoms, diagnosis and treatment records, insurance processing, invoicing, transmittance of billings, tracking and receipt of payments and account receivable management.
For the past several years, medical billing has been utilizing the paper-based technology. However, because of the entry of the computer technology and the internet, paper processing of medical billing has evolved and transformed into electronic billing and coding. Medical software companies have developed and upgraded their medical billing software to come up, promote and market their own effective health information systems to all players involved in the medical industry.
As a patient, your billing cycle process starts with the recording of the summarized details of your personal profile. Documentation of medical history record, tests done, symptoms observation and diagnostic physical examination procedures, proposed treatments, number of appointments and visits, your current health status and all other accurate and relevant data about you are entered into the patient's profile record.
Regarding how much the health provider will bill the insurance company; here is how they do it. A professional medical staff or medical coder translates into a number coding your diagnosis record based on the CPT and ICD-9-CM. By October of 2013, these formats will be phased out by ICD-10-CM. A five digit procedural code is assigned to your level of service info from the terminology database. These codes are their basis for insurance claims processing.
As soon as procedural and diagnostic codes are finalized, the medical biller will electronically format the insurance claim to an ANSI 837 file before sending to the insurance company or a clearing house. The claim gets processed by committees depending on the amount of claim involved. To verify info about eligibility of patient and the profile of the health provider, insurance companies use the rubrics procedure. Approved claims comprise an agreed percentage of the total submitted claim. Rejected claims are transmitted electronically through a remittance advice.
If your billing is rejected, your service provider will verify, make modifications and transmits back the claim again and the process goes on in accordance with the procedures on claim processing. Statistics show that there are a high percentage of rejected claims by insurance companies for multiple reasons. Some of them are due to patient's lack of eligibility and errors in the diagnostic and procedural codes used. There is a final legal remedy to contest a rejected claim which is the filing of an appeal before the appropriate authorities who have jurisdiction when a case is elevated to them for review of the decision.
When it comes to electronic eligibility and benefit of patient inquiry, the health care service provider performs this procedure thru its medical billing software. The format used is termed as X12-270 Health Care Eligibility and Benefit Inquiry. Response to the inquiry is also done electronically with the format of the X12-271 Response. As soon as you as patient are confirmed to be eligible, the healthcare services are provided. When the time for medical billing comes, the transmission is done through an X12-837 format to be responded by an X12-997. For the final adjudication of claim, the insurance company uses the response format of an X12-835.
The medical billing and coding department should be well oriented in all aspects of electronic billing and coding, all types of plans being serviced by insurance companies, and needs to be aware of the applicable laws, rules and regulatory requirements. Bill and Pay

I'm all about streamlining the workflow when it comes to QuickBooks - it minimizes data entry errors and saves time and money. I've started using a new service for invoicing and receiving payments - Bill and Pay. I had been using Intuit's Billing Solutions, which I still like, but clients have to pay by credit card. Intuit did add a new service, the Intuit Payment Network, so clients can pay by check, but it wasn't so simple to setup (since I already had Billing Solutions) and not as easy to explain to clients since it was two different services. However, still do-able. After hearing about Bill and Pay, though, I realized Bill and Pay offered features that Intuit's services did not and I could still use the Intuit Merchant Services, so I switched. So here are a few reasons why:
  • Bill and Pay has better integration (that was a real surprise) - I don't even have to open QuickBooks to receive payments - that saves time!
  • You can use Bill and Pay to help you stay on top of your receivables by sending reminder emails (automatically) -when the invoice is almost due and when it's past due; it's not automatic in QuickBooks and you can have only 1 template stored. You can choose how many days before and after the due date - I can't do that with QuickBooks.
  • If you accept credit cards, it's important to stay PCI compliant - especially with credit card fraud on the increase. This is one reason why QuickBooks will have you change your password every 90 days. With Bill and Pay, the credit card information is stored at Bill and Pay, not in your QuickBooks, so you've added another layer of protection for both your business and your customers.
  • While you can set QuickBooks and merchant services to automatically charge a credit card monthly it has to be the same amount every month. However, with Bill and Pay, the amount can vary as long as you have your customer's approval.
I will admit that it took a little time to do all the setup in Bill and Pay. The invoice template is not as flexible as the one that comes with QuickBooks and, right now, Bill and Pay has only 1 invoice template, but I hear they are working on adding more.
As before, paying online saves both you and your client time and postage.
And, if clients pay by ACH (check), you pay a flat check fee instead of a percent of the total invoice.
As a customer, you can view past invoices, view payment history, receive payment reminders, make a payment, and setup automatic recurring payments.
Because the invoice is online, you can pay even if you're out of town.

Most Accountants Are Not Tapping Into the Underlying Needs of Clients by Performing Needs Analyses

A recent survey from accounting experts has produced some interesting results. The survey asked local accountants to explain why clients came to them. Fifty-seven percent said clients need someone to "straighten out their records." Another 29% said clients came to them for tax return preparation and 14% said clients came to them to find ways to increase profits. What was interesting about these results was that they were all based on superficial responses made by clients. It seems that few the accountants perform needs analyses to determine their clients' underlying requirements.
According to expert in marketing accounting services, most clients do not seek out accountants for financial statements or tax returns. He pointed out that "most clients are looking for tax savings, increased profits, increase cash flow, and advice to help them reach these goals. The problem is that most don't put their needs in these terms. This is where a needs analysis can be of critical importance."
Needs analysis is a systematic process for helping people to decipher what they need and where their priorities like. Clients' needs and wants are often complex and usually one need conflicts with another. To untangle these needs and help clients prioritize for their businesses, a need analysis will ask them sets of questions. The questions are generally broken into three groups including questions that help clients focus on business goals, questions that help them discover new needs, and questions that help them relate their needs to their goals. Performing such analyses can help accountants generalize the needs of groups of clients to determine what services they should be offering and how they should be marketing those services.
One practitioner in the focus group pointed out that a competitor preaches that an accountant should differentiate his or her services by becoming a "trusted advisor" and by building a closer relationship with clients. This philosophy is mostly right because it emphasizes the role of an accountant as an advisor. Helping clients to understand their own needs is part of the advisory role. The best way to solve the true needs of clients is through a properly developed needs analysis that uses open ended probing questions. A needs analysis must often be tailored to a client's specific industry, with the ultimate goal being to help the client better describe his or her own needs. Reading trade magazines and keeping up with topics in the industry can help an accountant focus the client on his or her specific needs.
The utility of a needs analysis is that it helps accountants to help their clients, which is a winning situation for all parties. As an advisor, it is important that accounts recognize they are offering more than just number crunching services. Nearly 66% of Local Accountants Cede Market Share to Competitors by Foregoing Email Marketing

The explosion of social media services like Twitter and Facebook should be good evidence that people not only tolerate electronic communications, but they actually prefer them in many instances. In many ways, electronic communication is easier, which is why it comes as a surprise that two-thirds of local accounts in a recent survey admitted that they either do not use email to follow-up with online leads or that they only contact prospects offline.
In a focus group following the survey, two-thirds of local accountants admitted to not having access to email campaign software for such follow. The remaining one-third indicated that they used a paid or promotional product, such as an e-book, as a giveaway to solicit email subscriptions. Accountants who did not to engage in online marketing used postcard mailers and limited telephone follow-up as their primary marketing means. Eleven percent used email to communicate newsletters to existing clients only.
With internet advertising set to hit $50 billion by 2014, it is a tremendous oversight not to be involved, even if only on the local level. For those accountants who are not engaged in online marketing, particularly through email, market share is being lost to more aggressive competitors and major franchises that employ such techniques. One expert in marketing accounting services point out that, "the first problem is that clients anticipate such communications and have become accustomed to them. Internet searches now use geographic targeting, which means customers will search for services online and expect to be immediately connected to an email address or twitter account through which they can communicate with the provider. Clients are likely to pass up providers who do not offer such services in favor of those that do. The second problem with not engaging in electronic communications is that it makes the service provider look dated and out of touch."
Clearly, advertising and marketing are changing. The way that customers expect to communicate with a business is also changing. Potential clients are rarely looking to invest the time and energy needed to make a phone call or have a lengthy discussion. They are interested in having information delivered in a fast, efficient, electronic format. Email and other forms of electronic communication are no longer novelties, they are essential to the success of all businesses. Accountants should be making full use not just of email marketing, but also sites like Twitter, LinkedIn, Facebook, Tumblr, and more. A practitioner's online presence is quickly becoming the only one that matters.

The 31 Day Guide To Starting Your Own Accounting Business - Day 6 - What Are Your Revenue Streams?

Revenue Streams - where does your money come from, and where does it go?
Okay, if you've been reading this series, I'll bet you thought you were never going to get to the good stuff, did you?
Up until now we've been working to understand who your customers are and why they retain you as their accounting services provider. Along the way, we discussed the value they put on your services. I think it's about time you tried to figure out what they were truly willing to pay you for the services that they value.
When you segmented your clients by services you offer, and by the channel through which you reached them, you began to create a model for defining your revenue streams. Each channel, each segment, each service you provide can have it's own pricing mechanism, and from that, it's own pricing model and level.
You can have fixed price lists for fixed services, such as writeup, payroll or transaction recording, you can have contingent pricing for non-tax related advisory services, such as time and motion studies and yield management, or you can have value pricing for niche services such as cost segregation studies and loan packaging. Your choices are limited only by your imagination.
Your revenue streams will generally be composed of two different types, the unitary transaction model, where each transaction is negotiated as a standalone service, with a fixed term based on a named milestone, or they will be subscription based with recurring revenues resulting from an ongoing or open ended service, such as payroll or monthly accounting services.
The pricing mechanisms for each revenue stream will also depend upon whether your pricing is fixed or dynamic.
Fixed pricing are "list" prices is based on static variables, such as a product feature, the client segment, or the volume of services sold. Dynamic prices are based on market conditions and may be negotiated and based on factors such as yield, market, or auction.
Today, I want you to take the time to list the categories of revenue you offer. Your list can include everything from your monthly accounting and payroll services, to your quarterly and annual tax services, to your software and IT consulting services, and your project services such as loan packaging, cost segregation studies, of annual audits. If you offer physical products, such as computer or POS systems, make assets available for use, such as online payroll or SAAS (Software As A Service), lease assets or even lease employees, or even if you are licensing branded products, list each revenue model or stream.
Once you have defined each of your revenue streams, create yourself a spreadsheet of each streams contribution. In this spreadsheet, enter estimates of what you think your clients would really be willing to pay for a product or service (Value Pricing), and compare that with what they are currently paying. Note your current billing terms, and list how they would prefer to pay versus how you would prefer to collect. And, of course, calculate how much each of these revenue streams actually contributes to overall revenues.
Once you have completed this step, you should have a good picture of how your firm relates to and profits from your clients, and will have a good tool for understanding how to begin positioning your business and generating revenue.

Decoding Destiny - The Deciphering Power of Numerology

A code is a system of structural communication. The Genetic code houses DNA secrets of life. The Periodic Table houses the codes of all known 118 chemical elements and their atomic structures. Morse code transmits textual information through a series of dots and dashes expressed by either sound, light, taps or clicks. Our destinies are complex, containing multiple layers of energy, much like the layers of an onion. No one number or destiny component can define who or what we are, and what we are is basically an interwoven tapestry of collective and varying energies. Some energies in our destiny tapestry harmonize and blend, some energies are antagonistic and clash, just as some colors blend well together and others do not, or as some musical notes blend while others clash. The issue of importance is that each of our destinies is individual and unique to us. No two people are the same; no two destines are the same. In this we are each very special.

1. The letters of our full name
Letters are more than literary characters. They are, more precisely, symbols for specific energy fields akin to the pieces of a puzzle. Each letter is associated with a number and represents both a timeline of action and characteristics specific to it. When placed in certain arrangements (names), these letters create certain destinies.
2. Our full name as a whole (Expression)
Our names are not random. Nothing is happenstance in this universe. There is a divine design to all things and our names are no exception. Although we think of our names as being given by our parents, in the higher sense they are given through our parents by a divine power, a power that has formulated our destiny through a specific set of universal laws, karma (the field of action and reaction) being one of those laws. The names-these energetic puzzles-contain much of the framework of our fate. Our full name is referred to as our Expression. It houses all of our assets and liabilities as a person.
3. The single names comprising our full name
The single names comprising our full birth name are their own puzzles. Basically, our full birth names are a large puzzle housing smaller puzzles within it (single names), created by letters which are the smallest parts of the puzzle of the entire name. Therefore, our names are a puzzle (letters) within a puzzle (individual names) within yet another puzzle (the full birth name).
4. The vowels of our birth name (Soul and Material Soul)
Each of our basic needs, wants, desires and motivations are determined by the vowels in our full name at birth. The vowels are: A-E-I-O-U-Y. The numeric value of the vowels in our name constitutes our Soul. Add the Lifepath to the Soul and the Material Soul is created. Thus, there are two layers of desire energy in each of us.
5. The consonants of our birth name (Nature and Material Nature)
Our personality and manner of doing things are determined by the consonants in our name. Like the Soul and Material Soul, the consonants create a Nature and a Material Nature, the latter being a mixture of the Nature and Lifepath.
6. Our birth date as a whole (Lifepath)
The numbers associated with the day, month and year of our birth when added together comprise a major part of our Code of Destiny called the Lifepath, the script of our life. Our date of birth is, like our name, no accident. We do not have a certain destiny because we are born at a certain time, but rather we are born at a certain time because we have a certain destiny, as well as a certain name.
7. The numbers of our day, month and year of birth (Epochs)
Taken separately, the day, month and year of our birth each posses their own timeline and characteristics. These are called Epochs.
8. The combination of our name plus our birth date (Performance/Experience or PE)
When the Lifepath mixes with the Expression an energy is created that becomes the role we will give in life. This role is known as the Performance/Experience or PE for short.
9. Combinations of the natal data by addition (Pinnacles)
When the components of the birth date are added together in certain combinations, the resulting configurations are called Pinnacles. Like letters and names, they have their own timelines and characteristics.
10. Combination of the natal data by subtraction (Challenges)
When the components of the birth date are subtracted from each other in certain combinations, the resulting configurations are called Challenges. The have their own characteristics but share the same timeline as the Pinnacles.
11. Our age by year (Age Timeline)
Our age from year-to-year is a fluid part of the destiny puzzle. Its energies, marked by numbers and designated as the Age Timeline, have a major influence on the events, issues, problems, etc. for the year in question.
12. Our age in months only (Lifetime Monthly Timeline-LMT)
The total number of months of our life, counted singularly, comprise the Lifetime Monthly Timeline.
13. Our natal calendar year (Universal Timeline)
Our natal calendar year changes as the universal calendar year changes. The difference is that the energies of any given calendar year do not begin for us until the day of our birthday in that year. This is called the Universal Timeline. For a person born on January 1st, his natal calendar year will run concurrently with the calendar year. However, a person born on the 31st of December will not begin his natal calendar year until his birthday. Basically, he would begin his natal calendar year when the person born on January 1st would be ending his.
14. Linkage
Linkage is the continuous occurrence of the same number, numbers or number patterns in a numerology chart. Life linkage occurs when the numeric pattern in question begins at birth and continues uninterruptedly until death, i.e., for the entire life of the individual. Life linkage is often seen in the charts of famous individuals because the linking energy grows without interruption, thus allowing it to gain strength throughout the life.
15. Stacking
Stacking is the simultaneous occurrence of the same number, numbers or number patterns in a chart. While linkage creates continuity, stacking creates intensity. For example, if the number 8 appeared five times in different places in a chart during a given time period, it would have greater power than only one 8 during the same period.
16. Master numbers
Master numbers are multiple ciphers of the same single digit. There are two digit master numbers, such as 11, 22, 33, etc.; three digit master numbers such as 444, 555, 666, and four digit masters numbers such as 7777, 8888, 9999. Master numbers are extremely powerful, akin to nuclear energy. They can manifest positively or negatively. Generally, the more master numbers in a chart, the more power the destiny will exhibit.
17. Voids
Voids are missing numbers in the full birth name. They are like missing wiring in a car or holes in the fabric of an individual. For example, the letters A-J-S all equal a 1. If a person's full name has no As, Js or Ss, then he or she would have a 1 void. Voids are potentially problematic depending on if, when and where they occur in a chart. It would be wise, therefore, for a person to know of any voids in his name. When naming children, it is recommended the child's full birth name has no voids in order to increase the chances of a balanced life for the child.
Destinies and Snowflakes
What do our destinies and snowflakes have in common? As snowflakes are generally the same but specifically different, so our destinies are generally alike but specifically different. We all have a name, path in life, a role we will give in life, a desire base and personality, but the mixture of these ingredients creates a specific destiny unlike no other. No two people could possibly have the exact same destiny unless they were born with the exact same name in the exact same moment. Therefore, we are all unique unto ourselves. Furthermore, we are all quite complex, as are our individual destinies.
Summary
There is a Code of Destiny for each of us. That code is contained within the letters and numbers of our full birth name and birth date. The major components of the destiny code are discussed briefly in this article. Although generally the same because of our human status, we are specifically different because of our numbers. When one deciphers the codes of his name and birth date, the puzzle of his destiny will reveal itself, thus allowing him to expand his consciousness into the ever-expanding divine design of Reality.

There is more to it than that, of course, but those are all you need to start reading charts. (This leaves out computation of charts, for which you need astronomical background-- highly recommended.)
Imagine that you are a factory or maybe a business office. The planets are all the people who are involved in the workings of the factory or office. The zodiac signs are the roles they play, or rather, how they fulfill their roles in the business. The aspects tell how the people cooperate with one another. The houses are the various parts or departments of the building where they work.
To translate that into the human individual: Planets are dimensions of your being, each of whom have their own functions: Sun for ego, Moon for instinct, Mercury for communication, Venus for love, etc. Zodiac signs are powers that influence how the planets express themselves. Aspects are the relationships between the planets. Houses mark partitions in the horoscope that stand for fields of experience, like home, work, marriage, etc.

There's more than just natal astrology. Sooner or later, you'll want to explore other subjects such as forecasting, compatibility, divination and elections.
